REQUIRED documents:
* RECONCILIATION (2nd & 3rd Grade - NEW students):
1. Birth Certificate
2. Baptismal Certificate
* HOLY COMMUNION (NEW students - Grades 4th thru 8th):
1. Birth Certificate
2. Baptismal Certificate
3. Reconciliation Certificate or Letter of Completion from Parish your child
received his/her Sacrament of Reconciliation (ONLY if completed)
* HOLY COMMUNION (RETURNING students from last year - Grades 3rd & 4th)
1. Registration Form ONLY
* CONFIRMATION (High School) and RCIA (18 years old and above):
1. Birth Certificate
2. Baptismal Certificate
3. Holy Communion Certificate (if no notation on Baptismal Certificate,
ONLY if received)
NOTE:
If Sacrament(s) was received here at Santa Bernadita Church BEFORE APRIL 2015, Certificate(s) must be obtained from Our Lady of Lourdes Church
Registration Forms WILL NOT be accepted WITHOUT required documents.
FEE - non-refundable after September 9, 2024
